Vanderbilt University’s response to the COVID-19 pandemic is designed to sustain our important mission of education and research while maintaining the health and safety of our faculty, students and staff. Staff whose age is defined by the CDC has putting them at high risk or those that provide care for a family member or household member who has a health condition that falls within one of the CDC High Risk Categories may seek a workplace adjustment by utilizing this form. Staff who need to work from home related to COVID exposure or school closings for their children may also utilize this form.
The Centers for Disease Control (CDC) has identified several groups as those at increased risk for severe illness from COVID-19. Based upon available information to date, those at increased risk for severe illness from COVID-19 include: People over the age of 65, People with cancer, chronic lung disease or moderate to severe asthma; People who have serious heart conditions; People who are immunocompromised; People with severe obesity (body mass index [BMI] of 40 or higher); People with diabetes; People with chronic kidney disease undergoing dialysis; People with liver disease; People with Chronic obstructive pulmonary disease; People with cerebrovascular disease; People with cystic fibrosis; People with hypertension; People with neurologic conditions such as dementia; People who are pregnant; People with pulmonary thrombosis; People who smoke; People with thalassemia.
